Accessing Your Medical Record

You can access key parts of your medical record online, including test results and medications through Patient Access.

To register for Patient Access you can either:

  • Register online - you will be provided with your personal log in details and limited access to the site. Please bring photographic ID to the practice in order to allow us to enable access to further services.
  • Complete and return an application form (also available from reception) - Please bring photographic ID to the practice to be provided with your login details and password.

Sharing Your Medical Record

Increasingly, patient medical data is shared between GP practices, district nursing, hospitals and other secondary care providers in order to give clinicians access to the most up to date patient information.

The systems we operate require that any sharing of medical information is consented to by patients beforehand. Patients must consent to sharing of the data held by a health provider out to other health providers and must also consent to which of the other providers can access their data.
